Why is Sound Family Health a Complete Membership Practice? 

We founded Sound Family Health in April of 2009. Our goal, and the goal of our dedicated staff of support professionals, was to provide the best quality care, and put the needs of our patients first. These commitments have continued to guide our practices and our policies. Frequently, this has meant taking that extra step, often behind the scenes, to smooth out our patient's journey through the rest of the health care system. 

As time has gone on, America's health care system has undergone some significant changes. Independent primary care providers have experienced ongoing decrease in reimbursements. At the same time, there has been an increase in the costs of unfunded service mandates and reporting. Unfortunately, these trends are driving independent primary care providers like us out of business. 

Many independent physicians have chosen to become employees or contractors in large hospital-based organizations. Unfortunately, these physicians are no longer independent. The economic and corporate management interest of the large organizations often come into conflict with what the individual physician may believe to be in the best interests of the patient. As physicians, we did not want to be in this position. Because of this, we searched for an economically sustainable solution that would allow us to remain independent. 

Do I have to join Sound Family Health’s Complete Membership if I want to be a patient? 

Yes. Upon establishing care as a new patient, we will collect your annual membership fee. From that point forward, you will then have an annual “renewal month” in which you will receive a bill for the upcoming year. Your membership fee is always due by the last day of your renewal month. 

How much does the membership cost? 

Annual membership for patients ages 0-25 is $25 per year. 

Annual membership for patients ages 26+ is $180 per year.  

How do I cancel my membership? 

If you are not satisfied for any reason with the services provided by Sound Family Health, you may at any time withdraw your membership in our practice. SFH Complete Membership fees will be refunded if termination is within 90 days of enrollment/renewal at a prorated amount. 

Can the Practice cancel my membership? 

Yes. The Practice can terminate your status as a member by providing 30 days prior written notice if: 

  • You fail to pay the membership fee by the last day of your renewal month 

  • You fail to abide by the terms and conditions of your insurance coverage, if any; 

  • You fail to abide by the policies of Sound Family Health (i.e., inappropriate behavior, multiple no showed appointments) 

If the practice terminates your membership for any of these above reasons, your membership fee will not be refunded.  

Will SFH still bill my insurance? 

Yes. Sound Family Health will continue to bill your insurance company, and you are still responsible for the copays and deductibles. The membership model is separate from your current medical insurance; it is a yearly membership fee, which allows us to provide services to our patients beyond what medical insurance covers.  

Do I need health insurance if I’m joining the membership? 

You can establish with SFH whether you have insurance or are self-pay. Sound Family Health does not offer health insurance. The membership does not replace your health insurance. The membership will allow us to provide services beyond what medical insurance covers. 

Will my insurance pay for membership? 

No. We are unable to bill your health insurance for the membership fee. 

Should I join if I’m healthy? 

Yes. Sound Family Health will continue to provide preventative services, including exams, labs, and other screenings to keep you healthy and focus on preventing health problems from occurring. 

What does SFH Complete cover? 

SFH COMPLETE is designed to allow the providers and staff of Sound Family Health the ability to provide care beyond the covered benefits offered by Your health insurance. These extra benefits include:  

Same day visits, texting communications, extended appointments for multiple medical concerns, health newsletters, assistance in scheduling specialty appointments, phone triage, prescription refills, completion of administration forms, membership in gold cross to include nutrition benefits, life coaching, travel insurance, hotel bookings and travel concierge services.

Membership Payment is due upon scheduling your first appointment, then every 12 months.

SFH Membership is designed to allow the providers and staff of Sound Family Health the ability to provide care above and beyond the covered benefits offered by your health insurance. SFH Membership benefits and services are not covered or paid for by health insurance.

Your membership is in effect from the date your application is accepted by the Practice and will continue in effect indefinitely unless and until terminated by either you or the Practice.

SFH Membership Benefits and Services

These additional benefits and services are over and above the services covered by insurances.

  • SFH Membership allows your physician to have time slots built into their schedules for same day, acute needed appointments. These are on a first come, first serve scheduling basis, and are not always guaranteed. It’s recommended to contact us in the morning should you need to schedule.

  • SFH Membership allows you to communicate with the office staff and your provider via a secure texting platform, should you choose to use it. This is great for quick questions, scheduling appointments, requesting medication refills, and uploading medical pictures to our staff. 

  • SFH Membership allows you to schedule extended (longer) appointments when you have multiple concerns or conditions to discuss. This is different than an annual physical or wellness exam. Please request an extended appointment when scheduling, should you need to. 

  • Sound Family Health sends out quarterly newsletters with various health and clinic related information. This correspondence keeps members up to date with important information so you can make the best health decisions for you and your family. 

  • SFH Membership allows our staff to help with coordination of specialty appointments and radiology studies. This is dependent upon whether the specialty office allows scheduling through our office staff or directly with the patient. 

  • SFH Membership allows for phone communications (different than Telemedicine services) including treatment recommendations for non-complicated problems. This tends to be based upon your medical history and the current problem. This is especially beneficial for those who may be traveling or are out of town.  

  • SFH Membership strives to provide you with a years’ worth of refills for your prescription medications. This is dependent upon certain factors including the class of medication, your diagnoses, and your compliance with office visits. 

  • SFH Membership allows our office staff to complete different administrative forms for you without an office visit, when appropriate. Some of the forms included may be life insurance forms, FMLA, jury duty excusals, school athletic participation forms, disabled parking renewal forms, and medication appeals. Completion of these forms is not covered by your insurance.

  • Some of the benefits and services provided to SFH members are provided by Gold Cross USA. Gold Cross USA is the tradename of the American Association of Healthcare System Users and Medicos (AAHSUM LLC.). Your membership in Gold Cross/AAHSUM has been purchased by the Practice and is provided to you as part of Your SFH COMPLETE Membership, at no additional cost to you. If your Membership in the Practice is terminated for any reason, you will have the option to continue your membership in Gold Cross by paying a membership fee directly to Gold Cross. However, some gold Cross benefits are necessarily delivered with the assistance of the Practice and may not be available to you if you are not a SFH COMPLETE member. STATE INSURANCE - We ONLY ACCEPT United Healthcare Community Plan. If you have any other plan, you can not be seen in our office.
