New Patient Information

If you are new to Sound Family Health, welcome, we are glad you chose us to manage your healthcare.  Prior to coming to our clinic for your first appointment you may choose to print the New Patient Registration paperwork from this website, complete it and bring it with you to aid in a quick check-in process.  If you don’t understand a question, leave it blank and we will be happy to assist you in completing the form at the time of your appointment.

We ask that new patients check-in at least 15 minutes before that first appointment to complete the registration process.  All patients are required to present with a photo ID.

If you have health insurance, please bring your insurance information with you, as well as any co-payment which may be required per your insurance plan.  Uninsured patients must pay at the time of the office visit  and will receive a 25% discount.  We accept cash, check, Visa, MasterCard and Discover.  Once you become an established patient and are set up on our secure patient portal, you may pay any balance owing with any of the mentioned credit cards with the addition of American Express.

If you need to cancel or reschedule your appointment, please allow 48 hours notice to allow other patients to be seen in your place.

As a courtesy, our office will either call or send an email confirmation to remind you of your upcoming appointment. 

If you are running late to an appointment, it may be necessary to reschedule in order to keep the other patients on time, so please give our office a call.
